Independent Contractor Agreement
PLEASE READ THOROUGHLY (Users must select the check box to proceed to Section 2)
I understand that I will be sent an Independent Contractor Agreement and W-9 upon being selected for my first show. I also understand that if I do not fill it out and send it to the corporate office immediately, I WILL NOT BE PAID. That agreement shall remain in full force and effect until terminated by mutual agreement; however, the agreement may be terminated by CE Talent at any time upon my breach.

I also understand I will be sent a booking information document that will outline all necessary info needed to perform my job and a booking contract that will have all payment info. Both act as an extension of the Independent Contractor Agreement. If either is breached I understand that I may forfeit my pay and may not be used by CE Talent again. If I MUST cancel I will do so the moment I know in the hopes that the relationship with CE Talent will not be affected too negatively.
